Under Eye Filler Aftercare: How to Recover and Protect Your Results
Under eye filler, also known as tear trough filler, is one of the most technically demanding injectable treatments — and one where aftercare has a significant impact on results. The skin under the eyes is the thinnest on the face, making this area particularly prone to visible swelling, bruising, and temporary changes in appearance that can alarm patients if they are not prepared.
This guide covers everything you need to know about under eye filler aftercare: what is normal, what to do, what to avoid, and how long recovery takes.
Why Under Eye Filler Aftercare Is Different
The periorbital area (around the eyes) behaves differently from other injection sites because:
- The skin is thinner — changes in the tissue are more visible than in thicker-skinned areas
- There is less subcutaneous fat — swelling is more immediately noticeable
- Lymphatic drainage is slower in this area, making swelling take longer to resolve
- Bruising is common and more visible under the eyes than elsewhere on the face
- The risk of product irregularities (Tyndall effect) is higher in this area if too much filler is placed too superficially
For these reasons, under eye filler patients need to follow aftercare carefully and allow more time before assessing their final result.
Immediately After Under Eye Filler
In the first hours after your treatment:
- Noticeable swelling under both eyes — possibly more than you expected
- Redness around the injection sites
- Small bruises or pinprick marks where the needle or cannula entered
- Puffiness that makes the undereye area look temporarily worse than before
- Possible firmness that you can feel when touching the treated area
This initial appearance is not your result. Swelling under the eyes is always more pronounced immediately after treatment because the area has so little surrounding tissue to absorb the inflammatory response.
What to do immediately: Apply a cool (not ice-cold) compress gently around the eye area for 10 minutes. Do not press directly on the injection sites. Keep your head elevated and rest if possible.
Under Eye Filler Recovery Timeline
Day 0 to 1 — Peak Swelling
Swelling peaks in the first 24 to 48 hours. The undereye area may look significantly puffier than before treatment. This is normal and does not indicate a problem.
- Bruising begins to develop — colours from red to purple
- The area feels tender when touched
- Do not wear eye makeup or apply any products to the treated area
- Sleep with your head elevated (extra pillow) to reduce overnight swelling
Days 2 to 5 — Bruising Develops and Begins to Fade
- Bruising is often at its most visible on days 2 to 4
- Swelling begins to reduce noticeably from day 3
- The undereye area starts to look more like the intended result
- You can apply gentle concealer from day 2 if needed, using clean fingertips
Days 5 to 10 — Swelling Resolving
- The bulk of swelling has resolved
- Bruising fading to yellow-green tones
- Shape and placement of the filler becoming clearer
- Some residual puffiness may remain, particularly in the morning
Weeks 2 to 4 — Settling
- Swelling fully resolved
- Filler integrating into the tissue — any initial firmness softens
- You may notice some subtle changes in appearance as the product settles
- Final result becoming visible
Weeks 4 to 6 — Final Result
Under eye filler is considered fully settled at 4 to 6 weeks. This is the appropriate point to assess your results and contact your practitioner if you have any concerns.
What to Avoid After Under Eye Filler
First 24 Hours
- Eye makeup (mascara, eyeliner, eyeshadow, under-eye concealer)
- Rubbing or touching the treated area
- Exercise
- Heat exposure (saunas, steam, hot showers)
- Alcohol
- Bending down with your head lower than your heart (increases pressure and swelling)
- Contact lens insertion — wait at least 24 hours or as directed
First 2 Weeks
- Sleeping face-down
- High-impact exercise until bruising and swelling have resolved
- Eye procedures (lash treatments, eyelash extensions) until fully healed
- Vigorous facial massage
- Active skincare ingredients directly under the eye (retinol, AHAs) during recovery
How to Manage Swelling and Bruising Under the Eyes
Swelling reduction:
- Cold compress (gel pack wrapped in a cloth) applied gently around the eye area for 10 minutes, several times daily for the first 48 hours
- Sleep on your back with an extra pillow to keep the head elevated
- Stay well hydrated — dehydration makes the under-eye area appear more hollow and swollen simultaneously
- Avoid salt-heavy foods for the first week as sodium increases fluid retention
Bruising:
- Arnica tablets taken before treatment and in the days after can reduce bruise duration
- Arnica gel applied gently around (not on) the injection sites from day 2
- Avoid blood-thinning medications and supplements (aspirin, ibuprofen, fish oil, vitamin E) unless medically necessary
- Once bruising is surfaced, gentle vitamin K cream can help
Sleeping After Under Eye Filler
Sleep on your back with your head elevated for the first 2 nights. This reduces the pooling of fluid under the eyes overnight. From night 3, most patients can sleep in their preferred position, though staying off your front is advisable for the first week.
You may notice that swelling looks worse in the morning and reduces through the day — this is due to the overnight horizontal position. It should improve significantly by week 2.
Signs to Watch For
Most under eye filler recovery is straightforward. Contact your practitioner promptly if you experience:
- Pain that is worsening after day 2 (beyond tenderness)
- White or grey patches on the skin around the eye (vascular occlusion — rare but urgent)
- Any vision changes — blurred, double, or reduced vision (extremely rare but requires immediate emergency attention if it occurs)
- Signs of infection: increasing warmth, redness, and swelling after day 3
- Hard lumps that are not softening by week 2
When Do Under Eye Filler Results Appear?
The full result is visible at 4 to 6 weeks, once swelling has completely resolved and the filler has integrated. Trying to assess results before this point is misleading — the early appearance during swelling does not reflect the final outcome.
Under eye filler, when placed well, creates a smooth transition between the lower eyelid and the cheek, reducing the hollow or shadowed appearance of the tear trough. The result looks refreshed and rested rather than filled.
Frequently Asked Questions
How long does swelling last after under eye filler? The majority of swelling resolves within 5 to 10 days. Minor residual puffiness can persist for 2 to 4 weeks. Morning swelling (worse when you wake up) often persists for 1 to 2 weeks.
Is it normal to look worse after under eye filler? Many patients look temporarily worse in the first 2 to 5 days due to swelling and bruising. This almost always resolves to reveal the intended result. If you are still unhappy at 4 to 6 weeks when everything has settled, contact your practitioner.
Can I wear glasses after under eye filler? Light glasses are fine from the following day. Avoid heavy frames that press on the under-eye area for the first 2 weeks if possible.
Can I wear contact lenses after under eye filler? Wait at least 24 hours before reinserting contact lenses. If your eyes are still swollen or tender, wait until this has resolved.
How long does tear trough filler last? Under eye filler typically lasts 9 to 18 months. The tear trough area breaks down filler more quickly than some other areas because the skin is thin and the lymphatic drainage is active.
What is the Tyndall effect and can it happen with tear trough filler? The Tyndall effect is a bluish discolouration that occurs when hyaluronic acid filler is placed too superficially in thin-skinned areas — under the eyes being the most common site. It is rare when treatment is performed by an experienced practitioner using appropriate product and technique. It can be corrected with hyaluronidase.
