But what happens over the next two weeks matters enormously. The botulinum toxin is still localising to its target muscles, your skin is completing the microscopic healing process at each injection site, and your aftercare choices during this window can genuinely influence how clean, even, and long-lasting your results turn out to be.
This guide explains the key aftercare rules in detail — not just what they are, but why they exist — and sets realistic expectations for the results timeline, the two-week review, and how to get the most from each treatment cycle.
The Most Important Rule: No Lying Down for 4 Hours
If there is one aftercare instruction that practitioners emphasise above all others, it is this: do not lie down or tilt your head significantly forward within the first four hours of treatment.
The reason is product migration. Botulinum toxin is injected into precise locations to relax specific muscles — typically in the forehead, between the brows (the glabellar complex), or around the eyes. In the first few hours, before the toxin has fully bound to the local nerve endings at the neuromuscular junction, it can theoretically migrate beyond the intended treatment zone.
The consequences of migration depend on where the product moves to. Gravity is the relevant force here. Lying horizontally, or spending extended time with your head tilted forward — such as leaning over a desk or looking down at a phone for long periods — can encourage the product to drift before it has fully anchored. Standing or sitting upright with your head in a natural position is all that is required. You do not need to sit rigidly still for four hours; simply avoid lying down on a sofa or in bed, and try to keep your posture relatively upright.
After the four-hour mark, the toxin will have bound sufficiently to its target sites and the migration risk drops significantly. You can resume your normal resting positions from that point.
The First 24 Hours After Anti-Wrinkle Injections
Beyond the lying-down rule, the first 24 hours carry a few additional guidelines worth following carefully.
Do not touch, rub, or massage the treated areas. It is natural to be curious about how the injection sites feel, but applying pressure or friction to the skin can displace the product and increase bruising. Small raised bumps or mild firmness at injection points are normal immediately after treatment — these resolve within 20–30 minutes without any intervention.
Avoid applying heat to the face. Hot baths, steam rooms, saunas, and hot showers directed at the face all cause local vasodilation — the blood vessels widen, increasing blood flow to the area. This can worsen bruising and, in theory, contribute to product spread beyond the target zone. Lukewarm water for washing your face is perfectly fine.
Keep your skincare routine gentle. Cleanse lightly with a mild, non-stripping cleanser. Avoid harsh exfoliants, acids, or mechanical scrubs on the day of treatment and the day after. A simple, hydrating moisturiser and a broad-spectrum SPF are all you need. Applying make-up is acceptable from the day after treatment, but use a light touch — press product onto the skin rather than rubbing or dragging across the treated zones.
Avoid aspirin, ibuprofen, and other NSAIDs unless medically necessary, as these thin the blood and increase bruising risk. Paracetamol is fine if you experience a mild headache — which some clients do after forehead treatment, typically settling within a few hours.
Exercise After Anti-Wrinkle Injections
This is one of the most commonly asked-about restrictions, and for good reason — regular exercise is important and many clients are understandably reluctant to skip sessions.
The guidance is to avoid high-intensity exercise for at least 24 hours after treatment, with most practitioners advising 48 hours as a more comfortable buffer. The concerns are twofold. First, vigorous cardiovascular activity significantly elevates blood pressure and increases blood flow to the face, raising the risk of bruising at injection sites and potentially influencing how the product settles. Second, activities that involve lying face-down — certain gym exercises, yoga positions such as downward dog or forward bends — can create exactly the kind of head positioning that increases migration risk in the critical early hours.
Light walking on the day of treatment is generally considered fine. It is the high-intensity cardio, weight training, hot yoga, and contact sports that should wait until the following day or two. After 48 hours, you can return to your normal exercise routine without restriction.
One additional note: people who exercise very regularly and intensively tend to metabolise botulinum toxin more quickly than those who are less active. If you find your results wearing off faster than expected compared to friends who have the same treatment, this may be a contributing factor. Your practitioner can discuss appropriate dosing to account for this.
Alcohol and Anti-Wrinkle Injections
Alcohol acts as a vasodilator — it widens blood vessels and increases circulation — and also has mild blood-thinning effects. For these reasons, it is advisable to avoid alcohol for at least 24 hours before and after treatment.
On the day of treatment itself, alcohol will increase bruising risk at the injection sites. In the hours after treatment, drinking alcohol adds to the vasodilatory effects that aftercare is trying to minimise. A glass of wine the evening after your injections is unlikely to cause a significant problem, but if you have an important event coming up and want to minimise the risk of bruising, giving alcohol a wider berth for 24 hours before and after is a sensible approach.
Facial Treatments After Anti-Wrinkle Injections
A number of aesthetic and beauty treatments should be postponed after anti-wrinkle injections, both to protect the product and to avoid any interference with your skin's recovery.
Wait at least two weeks before:
- Facials involving massage, extraction, or steam
- Microneedling or collagen induction therapy

The honest answer is that the results do not appear instantly. You will not notice the muscles relaxing on the treatment table, and the day after your injections, your face will likely look largely unchanged. This is completely normal.

What If the Results Are Uneven or Not Enough?
It is not uncommon for results to settle unevenly in the first week or two. Because each side of the face is subtly different in terms of muscle strength, anatomy, and individual nerve response, one side may appear to relax slightly faster or to a slightly greater degree than the other during the settling phase.
Minor asymmetry in the first week is expected and almost always resolves as the treatment reaches its full effect at two weeks. Do not make any judgements about asymmetry before the 14-day mark.

With regular treatment, the targeted muscles can become conditioned to remaining relaxed, and some patients find they require slightly less product to achieve the same result after several treatment cycles.
Sun exposure and skin condition. Significant UV damage and compromised skin health can affect how treatments behave and how visibly effective they are at the skin surface.
